Once the source is established, the question becomes duration and response: what the cleanup protocol required, who was responsible, what triggered a response and what the records show was actually done within the relevant window.
By pattern and repetition. Condensation forms where a temperature and humidity differential exists and recurs in the same place under the same conditions, often at vestibules, cooler doors and loading areas. A spill is bounded and non-repeating, and the complaint history usually separates them.
There is no single number that fits every building. What is assessed is whether the operator escalated from its ordinary routine when conditions warranted, and whether the interval it chose was actually performed and recorded. A policy that never changes regardless of conditions is itself a finding.
It is one control among several. Adequacy depends on when it went out, where it stood relative to the approach, whether it was visible in the available light, and how long it stayed after the floor was dry. A sign left permanently loses the meaning it was meant to carry.
Installed condition and maintenance, yes: whether the floor actually falls to the drain as built, whether standing water is evident, whether the drain was kept clear, and whether repeated backups were recorded and acted on. Engineering calculations for drainage sizing belong to a licensed engineer.
Warm humid air meeting a cool slab or a cold-surface transition condenses on the walking surface. It shows up near loading doors, coolers, vestibules and mechanical spaces, and it recurs whenever the same conditions return. It is addressed as an installation, balancing and maintenance issue.
Hourly precipitation and temperature for the period before the incident, matched against the entry configuration, the matting provided and the inspection interval actually run that day. Foreseeable weather with an unchanged routine is a different picture from a sudden unforecast event.
